
MANILA, Philippines — In a major policy shift aimed at benefiting middle-class workers, the Department of Health (DOH) announced on Monday, February 9, 2026, that it will expand its Zero Balance Billing (ZBB) policy to cover shared “pay wards” in government hospitals.
The move ensures that PhilHealth direct contributors—those who pay monthly premiums through salary deductions or voluntary payments—can now enjoy better hospital accommodations without out-of-pocket costs.
New Policy Highlights
- Upgraded Accommodation: Direct contributors will no longer be limited to basic ward settings to avail of zero billing. DOH-run hospitals are now mandated to allocate “special areas” or shared pay wards for these patients.
- Amenities: Health Secretary Teodoro Herbosa noted that these upgraded rooms may include air-conditioning and more private spacing, moving away from the crowded common areas of basic wards.
- Scope: The directive applies to all 87 DOH-run hospitals nationwide.
- Exclusions: The ZBB policy still does not cover fully private rooms. Patients choosing individual private rooms will still be responsible for the corresponding room rates and service fees.
Addressing the “Middle-Class Clamor” The policy change follows a directive from President Ferdinand Marcos Jr. to ensure that the middle class, who consistently pay taxes and health contributions, feel the tangible benefits of the government’s health programs.
- Executive Push: Executive Secretary Ralph Recto recently met with DOH and PhilHealth leadership to push for “system-wide solutions” and expanded benefit packages for taxpayers.
- Recognition of Contribution: “We will give you additional benefits because you are contributing to the fund of PhilHealth,” Herbosa stated, acknowledging the huge clamor from workers for better privileges.
Implementation The DOH will release a formal memorandum circular to provide specific guidelines for hospital administrators. This expansion is part of a broader effort to modernize the Philippine healthcare system and make the ZBB program—originally highlighted in the President’s State of the Nation Address—genuinely accessible to a wider segment of the population.
By providing better accommodations at zero cost to those who fund the system, the government hopes to alleviate the financial burden of hospitalization on Filipino families while encouraging continued participation in the national health insurance program.
